The Oncology Physician Practice Coordinator (OPPC) is an integral member of the Smilow Care Center Delivery Team.
The OPPC is partnered with a physician to ensure the established plan for each patient is executed in the recommended time frame and meets the individualized needs of the patient and family.
The OPPC serves as the point of contract for daily operation isues inside the clinic and patient issues in the outside care settings.
The OPPC triages and communicates issues with the physician throughout the day while minimizing disruptions and delays to the clinic workflow.
Collaborates with intake team to schedule new patients and once plan has been established, take over as the consistent contact for the provider.

Serves as a clinical point person for identification and resolution or escalation of issues arising in day to day practice, anticipates delays and facilitates timely forward flow of patients during daily clinic operations.

Conduct post clinic wrap up with assigned provider to ensure the desired patient plan is executed.
3.
Completes individualized focused re-assessment and conducts post clinic wrap-up with assigned providers to ensure that the desired patient plan is executed, documents clearly.
4.
Performs pre-clinic preparation to ensure all scheduled patients are visit ready and identify appointments that require RN attendance along with Physician.
5.
Communicates critical patient data and assessment findings to providers, infusion nurses and other members of the care team as appropriate; ensures that handoff information is complete.
6.
Regularly reviews patient/family education plan created during new patient intake to ensure relevance and appropriateness.
7.
Administers unscheduled injections and performs unscheduled central line draws in exam rooms and medications 8.
Manages inbound clinical calls (eg, prescription renewals, clinical symptoms, etc) for all patients with an established plan.
9.
Assist provider with special procedure.
Provide support to patient and family during critical plan changes and communicate changes with delivery team.
10.
Assesses patients for referrals to supportive care services and follows through to ensure referrals have been executed.
11.
Intervenes in a timely manner to evaluate and resolve patient questions, concerns and reported symptoms.
Documents all communication clearly and concisely.
12.
Develops, prepares and facilitates a weekly interdisciplinary team meeting with all members of the Smilow delivery care team.
Identify any physical, logistical, emotional or financial barriers that may impact established plan adherence.
Make referrals for services as appropriate.
13.
Contributes to staff learning needs assessment; attends in-service programs and provides data for performance audit activities.
14.
Collaborate with clinical trial team to ensure all study patients have testing and appointments in accordance to the study protocol.
15.
Point person for all patients receiving oral chemotherapy.
Provide education and ongoing support for symptom management.
Assist specialty pharmacy with prior authorizations and communicate any financial barriers to the MAPS team.
Ensure patients receive medication on time and support patient during delivery issues.
16.
Performs other duties as assigned.
Qualifications: EDUCATION Graduate of an accredited nursing school required.
BSN preferred.
EXPERIENCE Minimum of 3 years Oncology experience required.
2-3 years Outpatient Ambulatory experience required.
Minimum 3 years medical surgical experience preferred.
LICENSURE Active CT RN license and OCN certification required within a year of hire.
SPECIAL SKILLS Excellent interpersonal and communication (oral and written) skills required.
Knowledge of adult learning competencies and patient teaching skills.
Ability to work autonomously and maintain flexibility.
